About the Role:
We are seeking obesity medicine physicians to provide longitudinal telehealth care for patients with obesity, and to collaborate with advanced practice professionals and registered dietitians towards exceptional team-based care. Work is remote, and schedules can be flexible.
We are currently accepting applications for a FTE (40 hours per week) Obesity Medicine Physician or Endocrinologist opportunity, who are licensed in GA.
This role will include participation in a rotating after hours call schedule with Form Health clinical leadership and will have a reduced clinical schedule the weeks of on-call coverage.
This is a Modified Full Time position based in a remote, telemedicine clinical setting. The physician will work closely with a supportive care team and have protected time for administrative duties.
What you will do:
- Provide direct patient care in a clinical setting, focusing on evidence-based obesity treatment and chronic weight management. The physician will have their own panel and will collaborate with Registered Dietitians.
- At this time, this role does not collaborate with APPs. In the future, this physician role will be responsible for chart reviews and conduct return visits for APP patients every third visit to ensure quality and continuity of care.
- Provide treatment planning and medical management support for complex cases or escalated concerns.
- Prescribe non-controlled obesity medications for patients under direct care
- Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team.
- Assist in the professional development of APPs by fostering clinical growth, offering feedback, and supporting continuing education. This role could include APP collaboration and supervision responsibilities.
- Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care.
- Attend relevant departmental meetings and contribute to quality improvement efforts within the obesity medicine program.
About You:
- MD or DO degree with board certification; must be an American Board of Obesity Medicine (ABOM) diplomate or board certified in Endocrinology
- Are licensed to practice and prescribe legally in GA.
