About the role
Accountabilities:
- Coordinate and oversee patient care requests, ensuring timely, compassionate responses and prioritizing patient needs and well-being.
- Serve as the central point of contact throughout the patient journey, coordinating communication among patients, providers, specialists, and care teams.
- Maintain accurate, complete, and current electronic medical records (EMRs) so that relevant information is readily available to care providers.
- Coordinate medical testing, procedures, referrals, and follow-up care while ensuring patients understand their care plans and next steps.
- Prepare personalized care summaries and medical briefs that clearly communicate treatment pathways and help patients feel informed and supported.
- Oversee the customization and distribution of Personal Medical Kits according to individual patient medical needs.
- Participate in the patient-centered on-call rotation, providing 24/7 patient support within 90 days of employment.
- Support the care team with unexpected or complex situations through calm, effective problem-solving and coordinated action.
- Ensure patient interactions and care coordination activities comply with applicable healthcare regulations, including HIPAA and confidentiality requirements.
- Contribute to additional patient-care initiatives and continuously support a compassionate, patient-first care environment.
- Associate degree in nursing, social work, or a related healthcare field with a strong focus on patient-centered care; an RN qualification is preferred.
- At least 5 years of healthcare experience, including demonstrated experience in patient care coordination and leadership.
- Proven ability to coordinate and integrate complex care plans while maintaining high standards of quality and continuity.
- Strong organizational skills, attention to detail, self-direction, and the ability to manage multiple patient needs effectively.
- Excellent written and verbal communication skills, with the ability to communicate compassionately with patients, families, and healthcare professionals.
- Strong creative problem-solving abilities and a commitment to improving the patient experience and care delivery.
- High integrity and a strong commitment to protecting confidential patient health information.
- Proficiency with Microsoft Office, Excel, and electronic health record systems.
- Ability to work with computers and medical or office equipment, review detailed clinical information, and remain focused during extended periods of computer-based work.
- Ability to lift up to 25 pounds and sit for extended periods as required by the role.
- Willingness to travel overnight for training or meetings and possession of a valid driver's license in good standing.
- Ability to work effectively in a remote environment while maintaining reliable communication, organization, and patient-service standards.
- Salary: $68,000–$73,000 USD annually.
