The Role
We're hiring a Nurse Practitioner to serve as the primary clinical point of contact for patients navigating their remote care journey.
In this role, you will lead clinical decision-making with a strong emphasis on guideline-directed medical therapy for heart failure, as well as adherence to evidence-based guidelines for type 2 diabetes and hypertension, including proactive and asynchronous medication titration based on patient vitals, symptoms, and trends. You will oversee patients, manage clinical escalations, and collaborate closely with the patient's physician or provider to optimize treatment plans, adjust medications, and determine the appropriate level of care. This role requires a high level of clinical autonomy, strong judgment, and close collaboration with Registered Nurses and Patient Success teams to deliver timely, patient-centered, and scalable remote care.
Licensure Requirements
- Required: Active Compact Multi-State RN license
- Required: Active NP licensure in at least two of the following states: TX, MI, NC, CA, PA, NJ, CT, AL
Licenses must be active and in good standing at time of hire.
Schedule Requirements
- Monday through Friday schedule (Flexible scheduling available, with a core shift schedule of 9:00 AM to 6:00 PM in Mountain, or Pacific Time)
