Position Overview
The Remote Exercise Physiologist plays a pivotal role in supporting patients enrolled in our pulmonary rehabilitation program. The role focuses on providing exercise programming, as well as helping patients navigate through the program through goal setting, progression tracking, and providing education and guidance on heart-healthy lifestyles. The Remote Exercise Physiologist will also support patients in managing their post-event recovery and work closely with a multidisciplinary team including nurses, exercise physiologists, and dietitians to provide comprehensive care for patients in various stages of recovery.
Responsibilities
- Patient Assessment and Monitoring: Perform comprehensive health assessments of pulmonary rehab patients, including physical fitness, medical history, lifestyle habits, and psychological wellbeing. Monitor patients' progress and adjust rehabilitation plans as needed, tracking metrics such as exercise tolerance, heart rate, and blood pressure.
- Personalized Care: Work with patients on exercises for our focus populations, typically older adults with pulmonary issues, over a telehealth/video conferencing platform. Implement exercise plans according to current professional and organizational standards, review patient progress, evaluate compliance and response to exercise, lead group exercise sessions, update group exercise programming in collaboration with the clinical operations team and medical director, and document all information accurately.
- Behavioral Coaching: Offer motivation and emotional support to patients, addressing concerns or barriers to following their rehabilitation plan. Utilize cognitive-behavioral strategies to help patients overcome challenges such as anxiety, depression, or lack of motivation, and identify barriers that would hinder program completion.
- Education and Counseling: Provide education on cardiovascular health, nutrition, and stress management, promoting heart-healthy behaviors. Provide counseling on lifestyle modifications including smoking cessation, dietary changes, stress reduction, and medication adherence. Promote regular program attendance, sign patients up for group education classes, and identify barriers to group education participation.
- Collaborative Care: Work closely with a multidisciplinary team including physicians, nurses, physical therapists, and dietitians to ensure comprehensive care for each patient. Provide regular updates on patient progress and challenges to the healthcare team.
- Data Collection and Documentation: Maintain accurate and timely documentation of patient assessments, progress, and communication. Record relevant data for tracking and reporting on program outcomes, patient goals, and adherence.
Position Details
This is a 1099 Contractor role, fully remote, with a schedule of Monday-Friday, 20-30 hours per week, between 8am-10pm ET. Compensation is $20-$30 per patient session (30 and 60 minute sessions available). Completion bonus available.
