About the role
Accountabilities:
- Determine appropriate rehabilitation assessments to better understand each Participant’s capacity for improvement, clarify needs, and coordinate assessments as required.
- Develop comprehensive Rehabilitation Plans in collaboration with Participants, families or caregivers, Rehabilitation Service Professionals, and Veterans Affairs Canada Case Managers.
- Ensure rehabilitation services and plans are needs-based, goal-oriented, safe, cost-effective, high quality, and aligned with program mandates.
- Act as the service integrator across medical, psychosocial, and vocational rehabilitation services, ensuring professionals work collaboratively toward shared outcomes.
- Empower Participants to identify solutions by exploring community-based interventions, resources, and services that can help them achieve their goals.
- Identify barriers, risks, and factors contributing to suboptimal outcomes, proactively developing and implementing strategies to address them.
- Coordinate communication between Participants, families, service providers, and other stakeholders to ensure continuity and alignment throughout the rehabilitation process.
- Support improved well-being across the six functional domains while helping Participants reduce or eliminate barriers to successful re-establishment.
- Bachelor’s degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, Human Services, or a related field.
- A recognized health professional designation, Certified Vocational Rehabilitation Professional (CVRP/CVRP-F) credential, or, in Quebec, registration as a Career Counsellor with the Ordre des conseillers et conseillères d'orientation du Québec.
- At least 3 years of direct experience providing medical, psychosocial, vocational rehabilitation services, or a combination of these areas.
- Experience supporting individuals living with mental or physical health conditions that create barriers to participation, independence, employment, or everyday life.
- Strong ability to develop individualized plans, assess needs, identify barriers, and coordinate multidisciplinary services.
- Excellent communication, relationship-building, problem-solving, and participant advocacy skills.
- Ability to work autonomously while collaborating effectively with multidisciplinary professionals and external stakeholders.
- Ability and willingness to travel within the region when required.
- Employment is conditional upon obtaining federal Reliability Status security clearance.
- French and English bilingualism is preferred.
- Experience working with employers, community resources, and training institutions is an asset.
- Current or former members of the Canadian Armed Forces are encouraged to apply.
- Remote work: Work from home in Quebec with minimal regional travel.
