Pharmacy Liaison

Job Purpose

Provides integrated off-site support to patients, pharmacists, and providers by delivering a full-service approach to total quality patient care. Works closely with patients to deliver a full continuum of medication adherence support using various tools and applications. Encourages patients of specialty clinics to utilize the hospital partner pharmacy to fully benefit from its superior patient care services.

Key Responsibilities

• Provide comprehensive care to hospital patients through custom medication management services
• Serve as an off-site member of the clinic team providing prior authorization, financial assistance, patient medication on-boarding, and refill/renewal support
• Educate prospective pharmacy patients on hospital pharmacy services and how they improve overall care
• Handle all medication needs of patients
• Grow the specialty pharmacy patient population by adhering to programs and processes
• Resolve insurance-related issues and complete benefit investigations including prior authorizations as an agent of the provider
• Review patient needs for financial assistance and secure available support
• Document patient information within hospital EMR systems

Work Location

Remote position with potential onsite clinic requirements. Must be located within 1 hour of Lakeland, FL.
