We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. This role requires at least 80% travel to support episodic in-home care operations across various markets.
The schedule is expected to be Monday – Friday 10am - 6pm local time with flexibility based on member needs, visit locations, and virtual acute care support.
Applicants must reside in one of the following states: Florida, Tennessee, North Carolina, Missouri, Texas, Georgia, Louisiana, District of Columbia
Active RN compact licensure required.
What You’ll Do
As a Pediatric Home Care Specialist & Acute Care Registered Nurse, you serve as the clinical bridge between home-based care and virtual acute care. You will deliver high-quality, family centered care in the home and community while flexing into virtual acute care nursing to support urgent pediatric needs.
In this role you will:
- Travel to members’ homes and community settings to deliver episodic, longitudinal, and urgent pediatric care.
- Build trusted, compassionate relationships with children and families, ensuring coordinated, culturally responsive care.
- Collaborate within an integrated care team including physicians, APPs, nurses, pharmacists, dietitians, navigators, social workers, and therapists.
- Conduct and document in-home assessments; perform procedures such as ear irrigation, point-of-care testing, IV placement and lab draws, wound care, and medication administration.
- Provide patient and family education, follow-up care, and care coordination using the Imagine Pediatrics platform.
- Flex into the virtual acute care RN role to conduct virtual assessments and manage urgent needs.
