The primary location for this role is remote, with quarterly travel. Expected schedule is Monday - Friday, 8:00am-5:00pm PST. Bilingual in Spanish highly preferred for this role. Washington state pharmacy license preferred.
What You’ll Do
The Clinical Pharmacist, as part of the interdisciplinary care team, provides medication management services for pediatric patients with chronic medical and behavioral health conditions focused on optimizing medication therapy, disease outcomes, and value of care. The Clinical Pharmacist is responsible for the day-to-day operations medication management services within the designated region.
In this role you will:
- Provide medication management services inclusive of medication access, medication education, medication adherence, comprehensive/directed medication management, and quality metric support for the designated region in alignment with the programs as designed by the medication management center of excellence.
- Assist with medication access management services between the patient, pharmacy, insurance, and medical provider through oversite and support of the region Medication Support Navigator.
- Provide accurate, thorough medication education, medication adherence assessment, and comprehensive/directed medication management through direct interactions with patients/caregivers, internal and external care team members, and works independently under individualized scope of clinical practice (where applicable).
- Document encounters in the patient electronic medical record, including closed-loop communication with patient care team.
- Promote the practice of evidence-based, cost-effective medication management by keeping abreast of new drugs, devices, and medical literature.
- Document/report adverse drug events (ADEs), medication allergies and medication errors per procedure.
