The primary location for this role is remote with quarterly travel for training.
Schedule will rotate to serve families needing later appointment times - current hours anticipated being 8:00am-4:30pm CT, 8:30am-5:00pm CT, 9:00am-5:00pm CT, and 9:30am-6:00pm CT. Potential expansions may require occasional shifts in PST.
Bilingual in Spanish highly preferred for this role.
What You’ll Do
As a Medication Support Navigator at Imagine Pediatrics you will work on the clinical pharmacy team to support our mission of increasing safe days at home for children with special health and behavioral care needs.
As an integrated care team member, the Medication Support Navigator focuses on improving medication related outcomes by supporting medication access needs for pediatric patients with chronic medical and behavioral health conditions. The medication Support Navigator works alongside the Clinical Pharmacist with primary responsibilities including maintenance of patient medication therapy records, accurately capturing medication histories, troubleshooting medication access barriers with community pharmacies and pharmacy benefits managers, and outreach to patient caregivers.
In this role you will:
- Manage, triage, and assign clinical pharmacy referrals to appropriate clinical pharmacy team member
- Support internal and external team members in the clinical pharmacy referral process based on patient needs
- Schedule patient/caregiver appointments for pharmacist-driven clinical programs
- Prepare charts in advance of Clinical Pharmacist appointments
- Communicates with pharmacies, insurance companies, and internal team members to resolve medication access barriers with the support of pharmacist where needed
