About the role

Accountabilities

    • Conduct ongoing medical record audits, including focused open and closed record reviews, electronic health record audits, ambulatory care reviews, and other quality-related assessments.
    • Evaluate healthcare services and clinical documentation against regulatory requirements, organizational standards, and applicable quality and performance measures.
    • Collect, organize, analyze, and enter clinical data into relevant databases while ensuring accuracy, consistency, and integrity.
    • Generate quality and performance reports and provide findings to appropriate clinical and organizational stakeholders.
    • Review data and documentation to identify opportunities for process improvement, patient safety enhancements, and reduction of potential errors or adverse events.
    • Support compliance with requirements and recommendations from regulatory and accrediting bodies, including CMS, The Joint Commission, and other applicable quality programs.
    • Interact professionally with physicians and other stakeholders by providing information, suggesting alternatives, and supporting data integrity.
    • Participate in internal studies, quality improvement projects, and performance initiatives within established timelines.
    • Identify and escalate serious events and incidents in accordance with established policies and procedures.
    • Maintain knowledge of departmental and organizational policies while contributing to a culture focused on quality, safety, learning, collaboration, and continuous improvement.
    • Requirements

      • Current Licensed Practical Nurse (LPN) certification and completion of an appropriate nursing specialty training program.
      • At least 3 years of professional nursing/LPN experience.
      • Experience or familiarity with medical record auditing, quality data collection, clinical documentation, or healthcare performance improvement is highly valuable.
      • Familiarity with CMS, The Joint Commission, quality reporting programs, and healthcare performance measures is preferred.
      • Experience with Core Measures and Get With The Guidelines® Stroke abstraction is highly preferred.
      • Strong ability to collect, analyze, reconcile, and interpret healthcare data with a high degree of accuracy.
      • Excellent critical-thinking and problem-solving skills, with strong attention to detail.
      • Effective written and verbal communication skills and the ability to interact confidently with physicians and multidisciplinary teams.
      • Strong teamwork and collaboration skills, combined with the ability to work independently and manage assigned responsibilities.
      • Ability to identify potential quality, compliance, and patient-safety issues and escalate concerns appropriately.
      • Comfortable working with electronic health records, databases, and other systems used for clinical quality reporting.
      • Ability to work a full-time 40-hour Monday-to-Friday daytime schedule in a remote setting.
