Your Role
As a Pharmacist with Evergreen Nephrology, you are responsible for ensuring the best clinical care for your patients. You will work with patients collaboratively with an assigned panel of physicians and care managers to manage the medication regimen for patients living with kidney disease and other chronic, complex diseases. This includes the opportunity to work with many different members of the interdisciplinary team including registered nurses, nurse care managers, health coaches, dieticians, pharmacists, psychiatrists, social workers, and even cardiologists and podiatrists – all within the Evergreen Nephrology family. While this position is fully remote, you must be able to work from 8:30am – 5pm in the central time zone.
Role Responsibilities
- Collaborate with physicians and care managers to manage medication regimens for patients living with kidney disease and other chronic conditions.
- Work as part of an interdisciplinary team to provide holistic, patient-centered care.
- Ensure the best clinical outcomes for patients through medication management and coordination.
