As a Care Coordinator, you are the patient's first point of contact. You ensure excellent patient care and satisfaction within a fully remote, telehealth environment, reporting directly to our Care Coordinator Manager. Responsibilities:
Answer current and prospective patient questions.
Triage inquiries to the appropriate medical staff across multiple platforms.
Ensure accurate data flow across platforms.
Troubleshoot patient technical problems within the internal dashboard system.
Help support our members' labs, pharmacy, and other resource needs.
Assist members with vulnerable mental health through safety planning and intervention.
Must-Haves:
A strong understanding and appreciation for the trans experience and a desire to increase access to gender-affirming care.
2+ years in a customer service role with a demonstrated history of excellent customer/patient/member service.
Great attention to detail in addition to data management and data entry skills.
Respectful and empathetic written and verbal communication.
Basic computer skills and comfort with learning new computer systems.
Comfortable interfacing with patients and colleagues within a fully remote environment.
Compensation:
- $60,000 Annual Salary
